This agreement is entered into between Kelita McCall (Doula) and client
Scope Of Services
I will provide professional birth doula services (the "Services") as follows:
One weekly prenatal appointment and consultations to discuss your birth preferences, pregnancy or birth concerns, pain management techniques, and prenatal care
Assistance in creating a birth plan outlining your preferences for labor and delivery
24/7 on-call availability from 36weeks of pregnancy until the baby's birth
Continuous physical, emotional, and informational support during labor and delivery
Guidance on pain relief techniques, such as breathing and relaxation methods
Advocacy of the Client’s preferences and needs to medical professionals during labor
One postpartum hospital visit, approximately 1-2 hours, to provide guidance and support with initial breastfeeding and postpartum recovery
Before Labor (Prenatal)
Kindly openly and honestly communicate your concerns, needs, and preferences for pregnancy, labor, and postpartum. We can discuss this during the prenatal visits. It’s also important to provide accurate information about your health status, medical conditions, and other details that could affect the pregnancy or labor to the best of your knowledge. I’ll need your updated contact details and emergency contact details.
During Labor
Please notify me as soon as you think you’re going into labor, even if you’re unsure whether you’re going into labor. This will give me enough time to prepare for your delivery and the birthing process. Kindly inform me whether you’d like me to be present there to support you throughout your birthing experience.
Hospital Regulations
Kindly inform me of any specific hospital policies or changes to policies that may affect my ability to provide support. Please also ensure that the hospital is aware of and consents to the presence of a doula during labor and delivery.
Service Limitations
It’s important to understand that I am not a medical professional. Therefore, I will not be able to provide services such as medical diagnoses and examinations or administering medications. I cannot make medical decisions on your behalf or override medical advice and hospital policies.
Confidentiality Release
Confidential information encompasses details such as your name, address, phone number, and other pieces of identifying information. It also includes medical or health information disclosed during our prenatal or postnatal visits and in our communication, as well as your birth plan, preferences, and concerns about pregnancy or labor. I will not disclose any such information without prior consent from you.
However, I may share relevant information with your healthcare providers to ensure coordinated and comprehensive care during labor and delivery. If a a backup doula is needed, I may share essential information with the backup doula, such as your birth plan and preferences, to ensure continuity of care.
Cancellation Policy
Any party may terminate or cancel the Agreement at any time by providing written notice to the other party.
Backup Doula
I will do my very best to be present during your birth. However, if I am unable to be there due to unforeseen circumstances, I may arrange for a qualified backup doula to support you through your labor. There will be no additional fees for the use of the backup doula, provided the conditions for their involvement are met as specified in this Agreement.
Force Majeure
Neither party shall be held responsible for any failure to perform under this contract due to causes beyond their control, including but not limited to natural disasters, extreme weather, accidents, and travel disruptions.
